The issues are sent free-of-charge, courtesy of our sponsors.  To facilitate these shipments we need the *name of the clinic or hospital, *the name of the receiver (i.e. MD, RT or rehab supervisor), *the complete address of the facility as well as the *clinic's phone number (for courier purposes only).
